ChartPath today announced a new partnership with Medlog Health and the addition of Medlog to the ChartPath Automation Marketplace, giving practices and long-term and post-acute care (LTPAC) facilities a more streamlined way to collaborate, escalate care when needed, and capture reimbursement for remote encounters. Medlog’s SBAR-based clinical messaging (Situation, Background, Assessment, Recommendation) enables facility staff to send the context providers need to resolve patient issues in the first message, minimizing unnecessary follow-ups and delays.
